REMINDER: Morgantown faculty, staff and students should begin taking daily wellness survey

Wellness Survey

To help the University monitor COVID-19 infection rates and community spread - even among remote/hybrid workers and students taking classes online - all faculty, staff and students should begin participating in the daily wellness survey Wednesday, Aug. 26. Responses will determine whether you can be on campus each day. If you do not respond, please do not come to any WVU campus. 

Full participation is expected, and participation will be spot-checked. Comprehensive and current information about the health of our community will help guide appropriate, data-driven decisions, including when it’s safe to bring more people back to campus. 

The survey takes about 1 minute to complete on a mobile device or computer. If you don’t have your email set up on your phone, you can also take the survey at wellnesscheckpass.wvu.edu. 

You should complete the survey seven days a week. Illness can develop at any time, and we want to ensure the University has the most current data possible.

You will receive an email every day at 5:30 a.m., as well as a reminder email around 9 a.m. Follow the provided link, which will be unique to you. Each day, you will be asked if you have knowingly been exposed to someone who has tested positive for COVID-19 within the last 24 hours. You also will be asked if, within the last 24 hours, you have had symptoms, including:

  • Fever. 

  • Cough.

  • Shortness of breath.

  • Muscle aches.

  • Headache.

  • Chills.

  • Sore throat.

  • Loss of taste or smell.

Once you complete the survey, you can generate an electronic pass on your personal device, indicating you are permitted on campus. You will be prompted to enter your WVU Login credentials and use Duo two-factor authentication to confirm your identity at this point. Once you have successfully generated a pass, you will receive a copy via email. Should you need to retrieve your pass later in the day, you can also get it at go.wvu.edu/pass.

All students, faculty and staff should be prepared to present this pass, which has a date stamp, if requested. Displaying your pass when asked signals respect for others and your shared concern for the well-being of our campus communities.

Additionally, WVU is encouraging the voluntary use of NOVID, a symptom-monitoring mobile app that will allow users to proactively make decisions based on their risk of infection.
