Community input is essential to determine the priorities that need to be addressed. The survey deadline is July 31.
The Patient Protection and Affordable Care Act requires 501(c)(3) tax-exempt (non-profit) hospitals to conduct a community health needs assessment and create an implementation plan at least every three years.
