Injury and incident report form available online

In order to streamline processes and provide a more accessible system, the Environmental Health and Safety team transitioned its Employee Injury/Incident Report form online.

All injury/incident reports should be submitted online through Campus Optics

It is critical that employees submit a report within 24 hours of an injury/incident.

The reporting system is accessible by phone or computer and allows reports to be submitted directly to both Environmental Health and Safety and Risk Management.

The digital reporting form is intended to improve and streamline current processes, making it easier to use and encouraging earlier reporting of incidents — ensuring faster response times and better tracking of workplace safety concerns.
