Employee information change forms moved to portal.wvu.edu

To provide additional information security protection to West Virginia University faculty and staff, the WVU Shared Services, Tax Services and Payroll teams recently moved several employee information change forms to portal.wvu.edu.

Active WVU employees who need to update their legal name, home address or payroll direct deposit account should access the related request form via portal.wvu.edu. 

The forms to update this information can be found within the general “My HR” tab (not under the “Restricted Apps” category). Note: Access to these forms does not require a connection to the WVU virtual private network (VPN). 

The “Legal Name Change” and “Change of Address” forms will automatically submit to WVU Shared Services once completed. 

The “Direct Deposit” form and supporting documentation should be submitted via LiquidFiles to payroll@mail.wvu.edu once completed.

Employees changing their legal name must provide an updated Social Security card showing their new name. Once the “Legal Name Change” form has been submitted, WVU Shared Services will reach out to the employee with additional instructions on submitting their updated Social Security card.

Employees visiting a WVU Shared Services Center storefront at Health Sciences, the Mountainlair or One Waterfront Place must present a WVU-issued photo ID (either employee ID or student ID) when making any changes. 

WVU cannot accept information updates submitted by anyone other than the employee (including updates submitted by a spouse or child).
