In an effort to simplify and standardize the process for students, faculty and staff to update their personal information with West Virginia University (e.g., legal name, date of birth, home address, Social Security number, etc.), WVU has transitioned to a single request form, effective today (July 15).
Students, faculty and staff previously may have been required to complete multiple request forms across various organizations (e.g., WVU Shared Services, the Office of the University Registrar, WVU Research Corp. and Health Sciences) when updating personal information, depending on the individual’s role or work location.
The updated Personal Information Change Form is available on the WVU Shared Services Community website. To access the form, you will need to log in using your WVU Duo credentials.
